You can either buy the DVD on Amazon (about ￥999 with free delivery for Prime members) or like in my case, watch it on Netlfix (if you have a video room with good internet connection).
This questionnaire is super simple with ridiculously easy questions. It can be given to students right after they finish watching to the film or even better: Challenge them to answer the questions as they watch to it.
